Treatment Consent Form Template
This tool is intended for use on tablets only.
What is this tool for?
This tool is designed to quickly obtain a patient's treatment consent in electronic form. A consent template is opened on a tablet, allowing the patient to review the agreement, check the required boxes, and sign using a finger or smart pen.
To complete the signing process, the provider and a witness must also add their signatures. After all signatures are collected, a PDF version of the consent form is generated and becomes non-editable.
The finalized document is automatically added to the Documents tab in the patient’s medical record.
How to use the template?
Click the plus icon (+) on the Patient Treatment Consent block.
A template library will open on the right side of the screen.
Select the desired template — it will be added to the Patient Treatment Consent block.
To begin the signing process, click on the template. This will open the consent form in full-screen mode for easier review and signature.

If the treatment consent form has already been signed, you can simply upload it.
How to create a template?
The treatment consent form template can be created by the administrator in the Group Settings/Progress Note section or by the provider in the progress note builder who has been granted access to create templates.

When creating templates, you can use different tags.
Tags
[ProviderName] - Current provider name.
[PatientName] - Current patient name.
[PatientID] - Current patient ID.
[__] - Text input field. Required to use.
[Required Checkbox | Contents of mandatory consent.] Required to use.
You will not be able to generate a PDF if you do not enter text in the field and do not click the required checkbox.
[Checkbox | Content of optional selection.]
Content not marked when signed does not get into the generated PDF.
